Doctor’s Referral

A Doctor’s Referral for Physical Therapy Evaluation and Treatment is required by most insurance policies prior to date of PT Evaluation.

Calling and requesting a PT referral from your child’s Pediatrician or Medical Specialist is our next step (the physical therapist can assist with this as needed)

Willow Bay will send you an access link to your new Willow Bay Patient Portal where all documentation and forms can be found for your child’s PT care.

Before scheduling Evaluation visit:

  • Intake paperwork must be completed and returned via Willow Bay Patient Portal

  • Insurance/payment processing must be verified by Willow Bay
